ADMISSION PROCEDURES
- The doctor’s admission order sheet - indicates specific requirements for the patient's case. In the absence of the doctor's admission order sheet or a doctor known to them, a walk-in patient may be admitted through the Emergency Room.
- Letter of Authorization (LOA) - for Company or HMO. Patients under the company need to present LOA while HMO patients need to provide HMO cards for verification of the coverage.
- Information requested will be written in the PIS and will be used for data gathering. This can be obtained from the patient on the day of admission or in advance to avoid inconvenience.
- All patients are given an ID band and a relative or significant other with a watcher’s ID to be worn for the duration of their stay at St. James Hospital.
- The patient will be asked to sign consent forms for hospital care, supply of medicines, limitation on outside diagnostic reports, the release of information to insurance companies and/or the patient's employer, and waiver of responsibility on the loss of valuables.
- Patients are referred to the Emergency Room (ER) if they do not have an admission order sheet from their attending physician. In such cases, the Resident Doctors will ask the patient for the name of his/her physician. The Resident Doctor may assign an attending physician to the patient's case if he or she does not have a personal physician of choice.
- The admission section accepts room queuing. The assigned room shall be confirmed on the day of admission and shall be held for the patient up to a maximum of two (2) hours prior to the patient’s scheduled time of arrival. However, if the patient fails to arrive within the indicated time of arrival, the room shall be assigned to other patients. The hospital also reserves the right to cancel queuing requests to give way to emergency and critical cases.
